If you are requesting that UPS leave the shipment at your location without a signature you will need to fill out the below document and send it to miket@hdtvsupply.com releasing HDTV Supply, Inc. and UPS from all liability for any loss or damage that may result from leaving this shipment w/o a receiving signature.
If the product is not where you have asked to leave it and/or is stolen, HDTV Supply, Inc and UPS will not reimburse you. If this document is not filled out and promptly sent to HDTV Supply your shipment will have to be signed for.
